This was one of those books where the timeline is not linear and the readers doesn't realize it.
Meghan was the one who pushed Caitlin. Caitlin was dating Ben and was jealous of the way that Ben spoke of Meghan. Caitlin pretended to be Nat, Meghan's high school friend, to get close to Meghan. Meghan figured it out when she saw her rings were missing. She was worried about Caitlin telling Ben, so she pushed Caitlin off the pedestrain bridge. Meghan also killed Caitlin with an overdose of insulin to keep her secret.
Luke, the other nurse, was the man who had been attacking women in the city.
