Nina wasn't the crazy one, Andy was the crazy one. He would often lock Nina in the attic as a twisted form of punishment for a perceived transgression. Nina plotted everything by bringing Millie into the house as a form of replacement. But when Nina got away, Millie was "punished" for leaving books out in the library. Only Millie didn't take it too kindly and ended up killing Andy. 

Nina told the police that Andy accidentally got locked in the room and his death was ruled an accident. The book ended with Nina and her daughter in California, yet recommending Millie's "services" to another abused woman.
