Casey's late husband, Len, was the serial killer who killed the missing women. Casey killed him when she found out by getting him drunk and pushing him into the lake.
Len's "spirit" inhabited Katherine and her husband kept her locked in the basement. But even before that, he was trying to kill Katherine for her money.
Casey and Boone ended up together. And Casey gave up alcohol.
