SPOILERS for The Silent Patient by Alex Michaelides

Wowzers. This one surprised me.

In a twisted timeline scenario, Theo was the man that Alicia saw standing outside of their house and following her.  He broke into their house and told Alicia that her husband was cheating with his wife, Kathy.  Alicia DID kill her husband because of the cheating.

Theo had also killed Alicia, because she recognized him from that night.  Once she started talking again she was a danger to him.  She wrote the truth in her journal and left it to be found after she was dead.
